Mt. Airy Learning Tree Registration Form

Here is another way to register for Mt. Airy Learning Tree classes. Below is a registration form that can be used in a variety of ways. You can print the form, fill it in, and then mail, fax, or phone in the information, or stop by the office to submit the form. The address and telephone numbers are at the bottom of this page. You can also type the information in the boxes from your keyboard and then print the form. At this time, you won’t be able to e-mail this form. If you need more than one form to register, you can click on the “Reset” button after finishing and printing the first form.

• Office hours are Monday-Friday, 9:30 a.m-1:30 p.m.
  
• Confirmations can only be issued if a stamped self-addressed envelope is provided with your registration form.
  
• Enclose a check, money order or MasterCard or Visa credit card number when registering. Checks are payable to MALT. Limited barter-based financial aid is available.
  
• Early registration is strongly encouraged. While we permit late registration, you may find that a class has filled or has been cancelled.
  
• Mark your calendar. Don’t miss a class you’ve been looking
  forward to.
  
• Refunds cannot be given after class has started.
  Please choose carefully. We must charge students $10 for returned checks.
  
• Rainchecks for a future class can be issued. Rainchecks are valid for one year. You must notify us before the first class session that you will be unable to attend.
  
• Your current address and phone numbers are important. If your class is postponed or moved, we want you to know.
  
• Directions to the class locations are on the locations page of the web site. Mark the location on your calendar. Classes in private homes are marked with an asterisk. You will receive the address when
  you register.
  
• Water Aerobics and Computer classes fill up very quickly.
  Register early.
  
• Read the course descriptions carefully. If you are uncertain about whether a course will meet your needs, call 215-843-6333. You may be referred to the teacher. Also previous students may have submitted evaluation forms which are available to read.
  
• Cancellation of classes is decided based on the number of registrations. Do not wait until the last minute. Your timely call will make the difference as to whether or not a class runs. We make every effort to notify you of cancellations. Please check your voice mail, the MALT telephone message or this web site.
  
• Snow and bad weather closings. MALT follows the School District of Philadelphia. If the schools are closed, so is MALT.
